FQHC Program Compliance and Grant Assistance

Since 2001, Eileen Tremaine has provided ongoing program compliance and grant assistance to a wide range of Section 330/FQHC health centers – from multimillion-dollar county health and hospital system to start-up community clinics.

 

Grant Development:

29 of 33 competitive HRSA Section 330 grants totaling $38.6 million have been funded, including two New Access Point (NAP) proposals.

FQHC-Look alike designation:

Prepared three initial designation FQHC-LAL proposals.

Annual and Three-Year Renewals:

Prepared 36 annual and 15 Service Area Competition (SAC) three and five-year competitive renewals for various Section 330 grantees. Two of the SACs were completed in 2018 after Compliance Manual has been integrated into the SAC application.

Compliance:

Provided 22 Pre-HRSA OSV Program Compliance Reviews and prepared numerous plans and responses to meet HRSA requirements. Developed a Continuous Compliance Program (CCP) for client FQHCs that supports the ongoing compliance efforts of health centers through expert guidance and capacity building. The CCP combines annual risk assessments, priority planning, quarterly team meetings, and performance tracking and reporting.

Changes in Scope (CIS):

Prepared over 55 CIS to add, remove, or change delivery of locations, services and target populations to HRSA approved scope of sites and services (Forms 5A & 5B).

Other Assistance:

Ongoing assistance to clients, including needs assessments, responses to special conditions, grant training, quality improvement updates, UDS preparation, Electronic Handbook support and education, and troubleshooting with HRSA Program Officers. 

Eileen’s successful experience with planning and program development in the areas of behavioral health integration, collaborative community-based organization contracting, and network development have strengthened and complemented this work.


FQHC Management Position & Executive Support

Eileen Tremaine has worked with Serve the People (STP) since its successful submission of the 2014 New Access Point (NAP). STP is located in urban Santa Ana, California, and has enjoyed extensive success through program and clinic site expansions with a current base grant totaling $1,989,204, and HRSA grants to renovate clinics sites for dental, mental health and vision services. In addition, by the end of the year STP will have four mobile clinics that provide primary care, dental care, behavioral health care, and vision services at homeless shelters throughout Santa Ana.

Mountain Valleys health centers

Eileen Tremaine served as Mountain Valleys Health Centers’ (MVHC) contracted Planning and Grant Officer from 2007-2019. The position required regular meetings and ongoing grant writing, management, and reporting work that provided extensive knowledge and understanding of issues, concerns, opportunities, and struggles facing both rural and urban FQHCs.

MVHC is a Northern California rural FQHC with eight clinic sites. Eileen worked with MVHC since 2001 when she started as their Site Manager and enjoyed significant success including federal grants to build two new health centers and the four HRSA grants to develop a regional health collaborative network to expand care coordination and to explore alternative payment models.


Rural Health Network Development

 

CA299 Health Collaborative

Eileen Tremaine was the contracted Network Director for the CA299 Health Collaborative, a rural health network of seven health care facilities including five Critical Access Hospitals, one FQHC, and one rural health center. The Collaborative is a collective of health care facilities located throughout northeastern California including Lassen, Modoc, Shasta, and Siskiyou Counties who work to facilitate improved patient care through collaborative opportunities including implementation of the health information exchange (HIE), care transition, and joint provider and allied health staff social media recruitment. Recent HRSA funding is supporting strategic planning for implementation of an Accountable Care Organization (ACO), behavioral health expansion and need assessment, and transition to full-scope care management across the service area.

Eileen and the Collaborative were very successful at securing federal funding to support the network’s efforts, including the following HRSA Office of Rural Health Policy grants:

  • Rural Health Opioid Program Grant - $250,000/year-three years (Awarded September 2018)

  • Two Rural Health Network Development Planning Grants – $200,000 (Awarded August 2018 & September 2012)

  • Rural Health Network Development Grant – $300,000/year-three years (Awarded September 2014


Marketing & Social Media Consulting

 

Tremaine Consulting Group is expanding our service line to include social media ad website maintenance consulting. With our proven and experienced media staff and FQHC expertise, Tremaine Consulting Group can provide with custom social media strategies, targeted advertising, and website enhancements that will expand health centers’ reach and brand awareness.

Based upon health center goals and needs for increased name recognition, sliding fee schedule promotion, and special event promotion, Tremaine Consulting Group will develop campaigns that fit the size, complexity and needs of the health center.

Madeline Johnson has been providing marketing and social media strategic planning and placement for the past three years and has worked with Dr. Elizabeth Stratte of Renew Dermatology, and Volition Beauty, a highly successful start-up skin care company that has placements on QVC and in Sephora stores nationwide. Madeline is a product of the University of San Francisco’s Entrepreneurship & Innovation Program and has spent the last three years working for Volition, the crowd-sourcing start-up beauty product company. Madeline has served as the lead website designer and social media manager. A contracted social media firm is utilized to amplify social reach to attract new clients and talent alike and to  establish direct relationships with the firm’s both existing and potential clientele to gather valuable demographic learnings.

Eileen Tremaine has extensive marketing, communications, and media relations experience as she has previously performed as the Marketing Specialist for Palomar Pomerado Health System, Escondido, CA; the Community Relations Manager for Sky Lakes Medical Center, Klamath Falls, OR; and as adjunct faculty (business communications, marketing) for Southwestern College, Chula Vista, CA.

Eileen and Madeline have joined forces to provide health centers with focused and cost-effective strategies and services that will expand brand awareness, promote access and service integration, and recruit personnel.
